An editable ‘Chinese Visa Application Form’ can be found as Attachment E in this pack. We have also included the ‘Chinese Visa Mail Application Checklist’ as Attachment F and the ‘Chinese Visa Payment Authorisation Form’ as Attachment G for those wanting to submit their application via post. Again, please check the website ‘Downloads’ page to make sure you have the most current information.

Further things required to complete the application are:

• A current passport – you will need to leave this with the Visa Centre
• A stamped ‘Letter of Invitation’
• A colour photocopy of the ID page of your passport
• A colour photocopy of any previous Chinese visas you may have
• A colour passport size photo to be attached to the application

On page 2 of the Chinese Visa Application Form (Attachment E) in section 2.1 you are required to state the ‘Major purpose of your visit.’ Choose ‘other’ and write, “to receive medical treatment.”

The next section 2.2 asks for ‘Intended number of entries.’ Choose the third option ‘Multiple entries valid for 6 months from the date of issue.’

In section 2.6 ‘Itinerary in China’ simply put the date of your arrival and the hospital’s address, which is:
